It is located at the top of the ‘MCA Info’ menu.
Fill this out to inform MCA decisions on tournament scheduling, setup etc.
Your name will be separated from the rest of the data which will be used at the aggregate level.
Members that fill out the survey will have their names entered into a random drawing at the Open for a fabulous but unknown prize.
